For changing your address. you have to write to your DP asking for a change of address, mentioning your client ID as well as the address to which you want to change it to and sign the letter. Along with the requisation letter which is signed by all the registered holders, you have to provide with a copy of telephone bill, electricity bill, rental agreement or a duly attested letter from a bank manager confirming the postal address. For changing your bank details, you have to write to your DP asking for a change in your bank details, mentioning your client ID as well as your changed bank details and sign the letter.
